To qualify for a First Time U.S Passport: 

1)You must be a U.S. born citizen or naturalized citizen of the U.S.
2)You never previously acquired a U.S. Passport.
3) You have had a passport but your previous passport is expired and was issued more than 15 years ago.
4)You have had a passport but you were under 16 when you acquired your previous passport.
5)You have had a passport but your passport was lost, stolen or damaged

Step 1: Obtain all materials. You must have all of the following items:

A) DS-11 form “Application for a U.S. Passport”. Please complete the application online HERE

B) Original or certified copy of your birth certificate, naturalization certificate, consular report of birth abroad of a U.S citizen or U.S. adoption decree.
C) Two (2) passport quality photos. Color photos, 2 by 2 inches in size and with a white background.
D) Check or money order: $170.00 payable to “U.S. State Department - Passport Services”.
E) Letter of Authorization with your original signature. Without this, applications will not be accepted.
F) Copy of your flight itinerary, airline tickets or cruise confirmation. Without proof of travel, the State Department cannot guarantee issuance of your passport by your travel date.

Step 2: Appear before a passport acceptor for the “signing and witnessing” of your application. (Passport acceptors can be found at ) Bring with you:

I) Proof of Identity (driver’s license must be over 6 months old)
II) Payment of $25.00 Passport Acceptance Fee payable to acceptance facility.
III) Items A through F from Step 1. Your passport application materials will be placed in an envelope and sealed. You must overnight the sealed envelope OR bring to our office, using an overnight service FedEx, UPS, Airborne, DHL to:

Your application will be hand carried to the State Department in Washington, D.C. for processing. We will notify you as to the status of your application by phone or email. When your passport is completed, we will use an overnight carrier for the delivery of your passport. Passports reported lost or stolen are invalidated and can no longer be used for travel.
If your passport is recovered after it has been reported lost or stolen, please submit it to the address listed on the back of Form DS-64. When you submit it, if requested - we will cancel it and return it to you. If not requested, it will be destroyed.
